Can somebody explain when and where the software decides to add tie-offs?
Tie-In and Tie Off stitches are sometimes called lock stitches. These locking stitches are added by the software at the beginning and end of an object if there is a distance of 2mm or greater between objects.
Beginning with the Version 5 BERNINA® Embroidery software you have the option of turning the automatic Tie-In and Tie Off function off. Select Object Properties and the Tie-In/Off tab to turn off the automatic function.
